Following the successful adoption of the NHS Prevention Pledge by all 16 provider trusts across Cheshire and Merseyside, a new programme is now underway to extend this approach into primary care networks (PCNs).
Developed by Health Equalities Group on behalf of NHS Cheshire and Merseyside’s Population Health Board, the Primary Care Prevention Pledge adapts the original model, used by secondary care providers, into a version tailored for PCNs and their affiliated GP practices. The aim is to provide a clear but flexible framework to embed prevention, tackle health inequalities, and support collaborative working across primary care settings.
The ambition is to expand delivery from 2025/26 onwards to a wider group of PCNs across all nine Places in the sub-region, building towards a more integrated, prevention-focused health system. Underpinned by six simplified commitments, the pilot focused on enabling primary care teams to formalise existing prevention activity, identify new areas for action, and integrate wider determinants of health into everyday decision-making. Commitments include strengthening links with social prescribing, fostering healthier workplace environments, and embedding brief interventions into clinical practice.
The pilot also emphasised peer learning and shared practice. Community of Practice sessions are now enabling participating sites to share what works, identify common challenges, and shape the model for future use. Early feedback has been positive, with participating teams citing the Pledge as a useful tool for improving coordination, driving staff engagement, and deepening the focus on tackling inequalities.
This approach mirrors the successful rollout seen in secondary care and signals a growing appetite within primary care to play a more structured role in population health.
